Complaint Process
Complaints against certified Harmonyum Practitioners
Harmonyum Education is responsible for investigating complaints and disciplining Harmonyum practitioners should a violation of the Code of Ethics and Professional Conduct for Harmonyum practitioners be determined.
Except for special circumstances, complaints must be in writing. HARMONYUM EDUCATION DOES NOT INVESTIGATE ANONYMOUS COMPLAINTS.
The information in the complaint must include:
- The name, address, telephone number of the person registering the complaint.
- The name of the person for whom this complaint is being registered if not the person registering the complaint.
- The name, address, telephone number of the person your complaint is against.
- Details of your complaint including:
- Dates
- Names
- Specific concerns of treatment provided
- Names of witnesses.
- Expected resolution regarding this complaint.
- Your signature, printed name and date.
The information provided in the complaint letter will be used in part to determine whether a violation of the Code of Ethics and Professional Conduct has occurred.

If Harmonyum Education decides to investigate a complaint against a certified Harmonyum practitioner, you must be willing to:
- Provide a signed declaration of the complaint
- Possibly testify to the allegations
- Explain your relationship with the certified Harmonyum practitioner
- Provide any other evidence in your possession
Harmonyum Education is not a law enforcement agency nor government agency. It is within the sole discretion of Harmonyum Education whether or not to investigate a complaint against a certified Harmonyum practitioner. The committee that will review the complaint consists of five members of Harmonyum Education.
INFORMATION COLLECTION, ACCESS, AND DISCLOSURE
Collection and Use of Personal Information. Harmonyum Education maintains the information you provide in your complaint.
Providing Personal Information is Voluntary. All information requested is voluntary; however, failure to provide the requested information may delay or prevent the investigation of your complaint.
Disclosure of Personal Information Provided. The information you provide may be disclosed in response to a court order, a subpoena or search warrant.
If it is determined by Harmonyum Education that a violation of the Code of Ethics and Professional Conduct has occurred, it could result in one of the following:
- A written warning and a permanent notation in the Harmonyum practitioner’s certification record.
- Suspension of certification for a period between six months to one year, at which time the Harmonyum practitioner may file a written petition for reinstatement of certification with Harmonyum Education. Reinstatement would also require the Harmonyum practitioner be current with continued education requirements for certification. Two suspensions will result in permanently revoked certification.
- Permanently revoked certification.
Harmonyum Education has the right to require additional remediation of the Harmonyum practitioner on a case by case basis.
